Penile girth is increased by adding a bulking substance to the subcutaneous tissue layers around the penile shaft. These layers are naturally thin, so only a limited amount can be safely added in a single procedure. Fortunately, human skin and tissue are highly elastic, allowing for gradual expansion over time.

With patience and proper care, the skin can stretch to accommodate substantial increases in girth over multiple sessions. While the maximum safe increase in girth during a single treatment is about **one inch**, further expansion is possible with subsequent procedures.

It’s also important to maintain balanced proportions. Excessive girth relative to the glans (head of the penis) can create an uneven appearance or, in some cases, make a circumcised penis resemble an uncircumcised one. While some patients and their partners appreciate this change, others may prefer a more natural look. No, we do not use silicone for penile augmentation—and we never will. However, we are one of the leading clinics for repairing complications caused by silicone-based enhancements.

In our professional opinion, silicone—whether in black-market injections or FDA-approved forms—is the worst material for penile enlargement. While injectable silicone may appeal to some due to its low cost and longevity, the risks far outweigh the benefits.

Unlike injected PMMA (polymethylmethacrylate), which can be chemically softened or surgically removed, injected silicone is permanent and remains in your body for life. This permanence means that if you’re dissatisfied with the results, you’ll have no options for reversal or alternative enhancements.

Additionally, silicone obscures ultrasonic imaging, preventing urologists from assessing internal penile health if issues arise. While silicone is marketed as “long-lasting,” it can also mean a lifetime of dealing with complications.

Silicone implants for penile enlargement are equally problematic. Decades ago, manufacturers of penile prosthetic implants for severe erectile dysfunction experimented with silicone to maintain penile length during prosthesis installation.

However, these attempts resulted in high infection rates, fibrotic buildup at the penile base (due to intense sexual activity), and, in some cases, severe Peyronie’s disease.

Unlike breast implants, which rest passively on the rib cage, the penis undergoes significant physical stress during intercourse and masturbation.

This makes silicone particularly unsuitable, as the risks of infection, tissue damage, and long-term complications far exceed any potential benefits.

For new patients, we recommend starting with procedures that preserve the most future options. For this reason, we also advise against the use of the only FDA-approved PMMA product, Bellafill, as an initial enhancement.

Once PMMA is injected into the subcutaneous penile tissue, it permanently reduces blood flow in the outer shaft, eliminating the possibility of future girth-enhancing options such as Fat Tissue or Dermal Graft Transplants.

Additionally, like silicone, PMMA interferes with duplex Doppler ultrasound imaging, making it difficult to assess internal penile health or administer treatments like PGE injections in the future.

Many overweight men mistakenly believe they have a micropenis because their penis appears smaller due to weight gain.
