Why We Start with HA Filler Before Bellafill®

The Problem with Jumping Straight to Permanent

Permanent injectable fillers like Bellafill® are less forgiving than HA. PMMA microspheres:

  • Do not dissolve
  • Can’t be reversed with an enzyme like hyaluronidase
  • May form nodules or granulomas if too much product is placed, or if it clumps early on

This risk is highest in the early healing phase, when your body’s inflammatory response is most active.

HA First, Then Bellafill®

Based on years of experience, we’ve found that starting with hyaluronic acid (HA) filler provides several advantages:

  • We can fine-tune shape and size before committing to permanence
  • The tissues become better structured and more uniform, which reduces clumping when PMMA is added
  • We see fewer issues with granulomas and nodules when Bellafill® is layered on top of a well-built HA foundation

Our foundational layering process works like this:

  1. Use HA filler to reach or approach your ideal girth in a reversible way.
  2. Evaluate the appearance and function over time.
  3. Once you’re happy, add Bellafill® in carefully measured volumes to “lock in” long-term results.

What Is BellaThick™ and How Does Bellafill® Work?

BellaThick™ is our name for non-surgical, off-label Bellafill® injections used for penile girth enhancement.

How Bellafill® Works in the Body

Bellafill® is the only FDA-approved PMMA-based filler. It contains:

  • Smooth, round PMMA microspheres (about 30–50 microns in diameter)
  • Suspended in a bovine collagen carrier

After injection:

  1. The carrier triggers a controlled inflammatory response, calling in cells that build new collagen.
  2. Your body lays down collagen fibers around the PMMA microspheres.
  3. Over time, the bovine carrier dissipates, leaving behind a stable collagen “scaffold” supported by the PMMA beads.

Because the microspheres are too large to be absorbed or broken down, the structure they support is expected to be mostly permanent when correctly placed.

What Results Can BellaThick™ Produce?

BellaThick™ is designed to:

  • Increase flaccid and erect girth

How Long Does Bellafill® Last in the Penis?

We believe Bellafill® is largely permanent, but we are honest that long-term data is still developing.

  • A large clinical study of 1,008 Bellafill® patients over 5 years showed an 87% retention rate, suggesting near-permanence for most individuals.
  • A 10-year study is in progress; as with any long study, data collection is challenging as patients move, lose interest, or receive additional treatments.
  • No other PMMA product in the world has comparable 5-year clinical data.

Because PMMA microspheres are not metabolized, we expect Bellafill® results to:

  • Remain mostly stable over time
  • Be affected more by mechanical stress (for example, aggressive sex or masturbation) than by natural breakdown

We provide specific post-procedure activity guidelines to minimize mechanical stress during early healing. This can reduce shear forces on the microspheres and may support longer-lasting symmetry.

Why Your PMMA Filler Should Be Done Only by a Board-Certified Urologist

Penile permanent filler is not a cosmetic spa treatment. It involves:

  • Complex penile anatomy
  • Erection mechanics
  • Lymphatic and vascular systems

Important Safety Note: Verifying Your PMMA Filler

In the United States, “PMMA filler” should mean Bellafill® — the only FDA-approved PMMA-based filler.

However, patients are sometimes told they received PMMA without documentation when they were actually injected with a banned substance.

Patients should always be shown the Bellafill® packaging and documentation.

If a provider cannot clearly identify the exact FDA-approved product used, that is a reason to pause and seek a urologic second opinion.

Who Is — and Isn’t — a Good Candidate?

Circumcision

We only perform permanent girth enhancement on circumcised men.

Adding girth to an uncircumcised penis often disrupts natural foreskin movement, leading to:

  • Phimosis
  • Infections
  • Other urologic complications

We do offer non-surgical cosmetic circumcision for men who plan to enlarge their girth later. At least 3 months must pass after circumcision before undergoing non-surgical penile enlargement.

Suprapubic Fat Pad

Men with a large fat pad above the base of the penis may appear smaller after girth injections, because more of the shaft remains buried.

These men may do better with a surgical pathway that combines:

  • Fat pad removal and abdominal contouring
  • Penile lengthening
  • Then, if desired, girth enhancement

Micropenis

In men with true micropenis, starting with permanent girth is rarely satisfying and often makes the penis appear even shorter.

We usually recommend:

  • Beginning with lengthening procedures (non-surgical or surgical),
  • Then assessing whether girth enhancement will create a balanced, functional size.
